Why Upload Your Insurance Policy

Your insurance policy contains the coverage details that determine what you should actually owe for medical services. By uploading your policy document, you give reKUPR the information it needs to verify whether your bill aligns with your coverage — turning a general analysis into a personalized one.

How to Upload

Both organization members (through employer-sponsored plans) and individual users can upload insurance policy documents. Navigate to your account settings or the insurance section of your dashboard, and select your policy document to upload.

Supported formats include PDF, JPG, PNG, and HEIC — the same formats accepted for bill uploads.

What reKUPR Extracts from Your Policy

Once uploaded, reKUPR uses AI to automatically identify and extract key coverage details from your policy document, including:

  • Deductibles — Individual and family deductible amounts
  • Copays — Fixed amounts for office visits, specialists, emergency care, and prescriptions
  • Coinsurance — Your percentage share after the deductible is met
  • Out-of-pocket maximums — The most you'll pay in a plan year before insurance covers 100%
  • Covered services — What your plan includes and any limitations or exclusions
  • Network requirements — In-network vs. out-of-network benefit levels

How Policy Data Enhances Bill Analysis

When reKUPR has your insurance policy on file, your bill analysis becomes significantly more accurate and actionable:

  • Coverage verification — reKUPR checks whether billed services are covered under your plan
  • Cost-sharing validation — Your copay, coinsurance, and deductible amounts are compared against what you were actually charged
  • Out-of-pocket tracking — Charges are evaluated in context of your deductible and OOP maximum
  • Network status checks — Billed amounts are compared against in-network vs. out-of-network benefit levels
  • Stronger findings — Discrepancies between your policy and your bill are flagged with specific, evidence-based recommendations

The Relationship Between Policies and Bill Analysis

Think of your uploaded policy as the baseline for comparison. Without it, reKUPR analyzes your bill using general medical billing rules and common patterns. With it, reKUPR can make plan-specific determinations — like whether a $50 copay should have been $30 based on your actual coverage, or whether a service was incorrectly processed as out-of-network.

Available for Everyone

Insurance policy uploads are available for:

  • Organization members — Employers can upload group plan documents that apply to all members, or individual members can upload their own
  • Individual users — Upload your personal or marketplace plan documents directly

Tips for Best Results

  • Upload the Summary of Benefits and Coverage (SBC) — This standardized document is the most useful for extraction
  • Include all pages — Make sure deductible tables, copay schedules, and exclusion lists are included
  • Update when your plan changes — Upload your new policy document at the start of each plan year or when switching plans
  • Check extracted details — Review the extracted coverage information to confirm accuracy after upload
